Mild encephalopathy with reversible splenial lesion (MERS) is a clinical-radiological spectrum characterized by acute encephalopathy due
to a reversible cause associated with a transient cytotoxic lesion affecting the splenium of the corpus callosum. It is considered a benign condition due
to its reversibility and excellent prognosis. MERS is characterized by altered consciousness, delirium, and seizures. The time between the prodrome and the
onset of encephalopathy usually ranges from one to seven days. We present a case of MERS due to its atypical presentation.

: Spontaneous spinal epidural hematoma (SSEH) is an uncommon but potentially debilitating spinal cord compression disorder. As the name implies,
it is an atraumatic neurological emergency characterized by the accumulation of blood within the spinal epidural space. It presents with symptoms associated
with acute neurological deficits. Its etiology remains controversial, yet it is hypothesized to be mostly related to vascular pathologies. Other risk factors
include anticoagulation therapy, coagulopathies, hypertension, cavernous angioma, and Paget's disease.

Spigelian hernia (SpH), a rare abdominal wall hernia with an incidence of 0.1-2%, occurs at the transition line between the transversus abdominis muscle
and its aponeurosis. SpH and especially low SpH can overlap with a concurrent ipsilateral inguinal hernia. A case of a 76-year-old male is reported here
where clinical examination failed to reveal the coexisting inguinal hernia that was diagnosed later on CT scan. The challenging diagnosis of SpH due to its
initial asymptomatic nature and its high risk for complications like obstruction and strangulation necessitates early elective surgery.

Autosomal dominant hyper-IgE syndrome (AD-HIES) caused by STAT3 mutations is a rare primary immunodeficiency disorder characterized by
recurrent eczema-like rashes, skin and pulmonary abscesses, and significantly elevated serum IgE levels. The purpose of this article is to highlight the distinct
clinical presentation, the importance of genetic studies in diagnosing AD-HIES and the role of bronchoscopy lavage in managing pulmonary complications.

De Garengeot hernia is an uncommon type of femoral hernia where the appendix is found within the hernial sac. The clinical presentation is often
atypical, ranging from asymptomatic to severe pain due to incarceration or strangulation. We report a rare case of a middle-aged female patients who
presented with a painful inguinal mass, which was initially suspected to be an incarcerated inguinal hernia. Preoperative CT imaging revealed a De Garengeot
hernia with the appendix and mesoappendix within the hernial sac. The patient underwent a successful inguinal approach for hernia repair with appendectomy
and mesh reinforcement.

Fibrolamellar hepatocellular carcinoma (FL-HCC) is a distinct histological subtype of hepatocellular carcinoma (HCC), accounting for approximately 1-2%
of all HCC cases. It typically affects young individuals without underlying liver disease, presenting unique clinical, histological, and molecular characteristics.
This case report describes a 25-year-old female with no significant medical history who presented with abdominal pain and was subsequently diagnosed
with FL-HCC. The patient's clinical course, diagnostic workup, treatment approach, and outcome are detailed.

Circulating microRNAs (miRNAs) have garnered substantial interest as minimally invasive diagnostic biomarkers for rheumatoid arthritis (RA). This
meta-analysis aimed to systematically appraise the diagnostic performance of circulating miRNAs in the detection of RA. A diagnostic test accuracy (DTA)
meta-analysis was conducted, synthesizing data from 20 eligible studies evaluating circulating miRNAs as diagnostic tools for RA. Pooled estimates
for sensitivity, specificity, diagnostic odds ratio (DOR), positive likelihood ratio (PLR), and negative likelihood ratio (NLR) were derived using a random-effects
model.

Early childhood presents multiple risk factors for developing chronic otorhinolaryngological diseases, which frequently follow a protracted,
relapsing-remitting course with exacerbations and complications. Increasing environmental stressors, rising allergy prevalence, and the emergence of
drug-resistant microbial strains have contributed to the growing burden of nasopharyngeal tonsil disorders, particularly among children, both in Russia
and globally.

Heart disease is the leading cause of non-obstetric mortality in parturients, accounting for 15-20% of maternal deaths (1,5).
The presence of severe heart disease is considered a contraindication to pregnancy.
The management of these patients is based on a planned multidisciplinary approach, pre-, peri- and post-partum.

CSVT is a rare manifestation in sickle disease. It effects 5 people in 1 million each year and the risk is more in new born and takes up to 3 out of 3 lakh children
up to the age of 18 yrs (1). Sickle cell disease children are more amenable than sickle cell traits. Increased clotting mechanism is responsible for this
manifestation. We present here two cases who have presented with different clinical manifestations. Both of them are known cases of sickle cell disease with
different presentation and here we discussed the mechanism and management. we lost one case with haemorrhagic infarct and gross midline shift and the other one recovered well and discharged with complete recovery.
